This office is a MEDICARE participating provider. If you have a supplemental policy, then we will include this information on our claim to Medicare.

If you do not have a supplemental policy, then the 20% portion of the Medicare fee schedule will be collected at the time of visit.

We are also participating providers with BLUE CROSS, BLUE SHIELD, UNITED HEALTHCARE, AETNA, AvMED, CIGNA PPO, and several other insurance companies.
